Make an Appointment

We are happy to book an appointment for you. This is not an automatic process. Once we receive you request, we will contact you to confirm a date and time that is as close to your request as possible. This form should only be used for requesting appointments that will take place at least 2 full business days after the time of the submission of this form. If your appointment has not been confirmed within 24 hours, please feel free to contact the practice by phone to confirm. ***Please note that our online appointment form is only for elective appointments, not emergencies. Call the clinic directly if the appointment is for an ill or injured pet.***

We require a deposit of $50.00 at the time of scheduling for all new client appointments as this reduces no-shows and last-minute cancellations that impact our ability to care for current and new patients. This deposit will be applied to your first visit. If you need to reschedule or cancel your appointment it must be done a minimum of 24 hours prior to the scheduled appointment, or your deposit will be forfeited.
